💡 The short answer. Not everyone, but most patients. If 3 to 5 mm or more of bone remains beyond the root tip so the implant can be held firmly, and there is no acute purulent infection, the implant can be placed at the same time as the extraction. What is different about immediate placement?

The usual approach waits 3 to 6 months after extraction for the bone to heal, places the implant, then waits another 3 to 6 months for osseointegration before the prosthesis goes on: 9 to 12 months in total and 8 to 12 visits. Immediate implant placement puts the implant straight into the extraction site and fills the empty space with graft material. The waiting period after extraction disappears, bringing it down to about 3 months in total and 4 to 6 visits.

It is not only the period that shrinks. Because it is placed before the bone has shrunk after extraction, it is favourable for preserving the shape of the gum and bone, and the surgery is done in one.

When is it not possible?

When there is almost no bone beyond the root tip so stability cannot be achieved; when the buccal bone wall is largely lost; or when acute infection is not under control. In those cases we go to the staged approach, grafting after extraction and waiting. Uncontrolled diabetes, heavy smoking and severe bruxism are not absolute contraindications but do affect success rates, so they are checked together at consultation.

Which implant is used?

Bone immediately after extraction has a wide opening and is soft, so a design that grips firmly even in a limited site is needed. The Straumann BLX we use as standard is a fixture designed to gain primary stability through a root-form body and deep threads, using the Roxolid alloy and the hydrophilic SLActive surface. The manufacturer’s data give 48.3% bone-to-implant contact at 4 weeks and a 10-year cumulative survival rate of 98.23%. Seongmo Dental Clinic applies a 10-year warranty to Straumann BLX.

Do I get a tooth the same day, and what about cost?

It depends. Where primary stability is sufficient and the site is visible, such as the front teeth, a temporary tooth can be placed the same day so there is no gap period. For molars, which take large chewing forces, it is safer to wait until osseointegration is complete before the prosthesis. The final prosthesis is made in-house with Primescan and Primemill.

Implant cost is determined by the fixture brand and the extent of grafting; no separate item is added for immediate placement. Is the success rate of immediate placement lower?

Can a tooth with infection have an immediate implant?

Chronic inflammation can be placed after the socket is thoroughly curetted. An acute state with swelling and pus is settled first.

Does immediate placement give me a tooth the same day?
